Service Rally in the Valley

Student Participant Registration Form

The Service Rally in the Valley will be April 26, 2026. FBCVM will be partnering with other local churches to do service projects in our community. This registration form is to be filled out by the parent/guardian, it is for students (6th - 12th graders) who would like to participate as a volunteer in the Service Rally and will not have their parent/guardian accompanying them during the event. In addition to completing this registration form, please click the button below to access the release form for the Service Rally on the Valley. The release form must be signed by the parent/guardian of the student in order for them to participate. Participant forms need to be completed by April 17th, 2026. Thanks!
