Student Participant Registration Form

The Service Rally in the Valley will be April 26, 2026. FBCVM will be partnering with other local churches to do service projects in our community. This registration and release form is for students (6th - 12th graders) who would like to participate in the Service Rally and will not have their parent/guardian accompanying them during the event.
